Overview
Under supervision, provides social services to clients/patients, such as recipients of public assistance, food stamps and/or medical assistance, adults receiving or needing institutional care or protective services.
Responsibilities- Identifies, develops and implements social service plans for such families/individuals – determine the eligibility for these services and may perform counselling and investigatory activity related to the work.
- Candidate will be fully trained with the department
- Requesting candidates with direct experience placing people in nursing homes, case management, and experience sending out referrals Careport and EPIC experience preferred
