Community Health Worker

Overview

Job Description

Works as a liaison to grant support to the vulnerable members of the community, addressing material and health challenges. Maintains a caseload of members and provides referrals to various support services by conducting assessments and in home visits. Serves as a role model and will identify and initiate referrals to social service programs, serve as main point of contact for the member and support the implementation of various care activities.

• Serves as a mentor for Community Health Workers
• Provides oversight in care of members to improve health outcomes and promote well-being
• Acts as a resource for CHW and members
• Provides support and basic health and education about material and general health care and community resources
• Conducts one-on-one health education, community outreach, regular in-home visits and service coordination
• Provides various supports, services, entitlements and benefits for members
• Supports members in developing health management care plans
• Empowers members by promoting effective approaches, problem solving and lifestyle changes
• Works to establish positive and supportive relationships and effective work with a diverse group of members
• Delivers coaching and mentorship on chronic health conditions and self-care
• Enters client data into software on a daily basis
• Advocates for members and their families and collaborates with the care team as needed

• High School Diploma or equivalent required
• 1-3 years of relevant experience, required
