We’re seeking a Licensed Master Social Worker (LMSW) to support patients’ psychosocial needs, discharge planning, and care coordination. You’ll work closely with a multidisciplinary team to ensure patients and families receive the highest quality support during their hospital stay.
What You’ll Do- Conduct biopsychosocial assessments and implement care interventions
- Provide supportive counseling to patients and families
- Coordinate discharge planning and connect patients with community resources
- Document patient care accurately in EMR systems
- Participate in multidisciplinary rounds and advocate for patient needs
- Screen for abuse and comply with mandated reporting protocols
- Master’s degree in Social Work
- Valid LMSW license (NYSED)
- Minimum 1 year of experience (hospital, inpatient medicine, surgery, or rehab)
- Strong communication, empathy, and teamwork skills
