Overview
Salary Range: $35k–$40k (varies depending on years of experience)
Our client is seeking a Certified Nutritionist to provide personalized nutrition counseling and support to patients in an outpatient healthcare setting.
The ideal candidate will help patients manage chronic conditions, achieve wellness goals, and adopt healthy lifestyle habits through evidence-based nutrition guidance.
Key Responsibilities- Conduct assessments and develop individualized nutrition plans tailored to patient needs
- Provide counseling on healthy eating, weight management, and disease prevention
- Support management of chronic conditions such as diabetes, hypertension, and cardiovascular disease through nutrition interventions
- Lead nutrition workshops, educational programs, and community outreach activities
- Maintain accurate and timely documentation of patient care and nutrition plans in the health record system
- Provide patients with resources related to food security and community nutrition programs
- Prior experience in an outpatient healthcare or clinical setting required
- Strong communication, counseling, and interpersonal skills
- Knowledge of evidence-based nutrition practices and patient education strategies
- Work directly with patients to impact health and wellness outcomes
- Collaborate with a multidisciplinary team in an outpatient healthcare setting
- Engage in community education and health promotion initiatives
