Responsibilities- Application / Assessment / Intake:
Answers calls from individuals seeking information and assistance related to chemical dependency issues and related problems. Provides basic information regarding chemical dependency and service availability, offers crisis intervention if necessary, completes the pre-screening form, and establishes intake appointments as requested. Interviews applicants who request admission and completes designated counseling sections of the Integrated Assessment Form. Provides an overview of the nature and goals of treatment, explores treatment options, and explains levels of service.
Identifies problem areas to be treated and lists them as part of the Clinical Formulation / Integrated Summary of the Integrated Assessment Form. Indicates applicant’s strengths and completes Axis I, II, IV, and V. Provides basic information to the program Physician or Physician's Assistant and completes the medical sections of the Integrated Assessment Form. Recommends treatment based on needs, eligibility, and appropriateness for admission to a modality and level of care.
Provides referrals to those not accepted into treatment. Recognizes possible signs and symptoms of co-occurring psychiatric disorders and recommends evaluation by a psychiatrist to medical staff. - Treatment Planning:
Reviews information from the Integrated Assessment Form and presents it to the assigned interdisciplinary team at case conference. Interviews patients and coordinates efforts to develop an Initial Treatment Plan that states problems, goals, activities, responsibilities, and timelines. Completes the Bio-psychosocial Addendum and creates comprehensive diagnostic assessments. Monitors patient progress and reviews the Treatment Plan to ensure relevance. Completes treatment reviews as required by patient needs and regulatory requirements.
Updates treatment plan and obtains necessary signatures. Completes HIV Risk Assessment semi-annually for outpatients and provides follow-up per policy. Initiates discharge planning and finalizes aftercare plan with referrals; completes Discharge Summary at time of release. - Case Management:
Functions as a case manager with responsibility for treatment oversight, coordination of referrals, and resolving patient problems. Completes semi-annual Financial Assessment; follows up on billing issues and balances. Serves as primary contact to insurance carriers and manages entitlements. Monitors toxicology testing and records results. Advocates for patients to access necessary services. Collaborates with family or significant others, monitors attendance, and follows up on missed visits.
Schedules appointments and provides referrals. Initiates and facilitates movement between levels of care with orientation and support. - Counseling:
Establishes therapeutic rapport and provides orientation to treatment, goals, and levels of care. Delivers individual counseling to identify problems and develop solutions; sets goals and supports recovery. Provides crisis intervention as needed and adheres to policy if patients seek to leave AMA. Facilitates group counseling and engages family members when appropriate. Delivers gender-specific treatment and adapts techniques for women and patients with co-occurring disorders. - Patient
Education:
Identifies patient needs and develops educational activities and materials. Provides individual education and group seminars on chemical dependency, healthy living, and recovery. Conducts outreach to educate the public about treatment availability. Helps patients access resources. - Environment and Emergency Situations:
Monitors patient whereabouts and behavior; enforces unit policy. Reports emergencies or unusual situations with outcomes and recommendations. Completes incident reports as necessary. - Documentation:
Records significant occurrences and documents activities, services, and referrals. Completes notes promptly and professionally in relation to treatment goals. Obtains required consents/releases and ensures they are processed correctly. Prepares documentation for third-party payers and maintains open communication. Completes departmental statistical reporting and required forms from funding sources. - Performance Improvement:
Participates in peer chart reviews, mentoring, and PI activities. Attends PI meetings and contributes to department-wide quality initiatives. - Professional Development:
Participates in planning and delivering in-service training; engages in professional organizations and ongoing education. Attends staff meetings and relevant conferences to enhance professional skills.
- Master's Degree in related field required.
