** The base pay scale for this position is $ - $. In addition, this position will be eligible for additional benefits consistent with the role. The salary of the finalist selected for this role will be determined based on various factors, including but not limited to: scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation.

** What you will be doing
** The Case Manager incorporates the case management process of assessment, planning, implementation, coordination, monitoring and evaluation into daily practice to manage the patient's broad spectrum of needs across the continuum of care. The Case Manager applies clinical expertise and core knowledge domains of case management into daily practice. Data is interpolated from patient/ family assessments, the interdisciplinary care team and medical record to evaluate the patient's inpatient plan of care, progress along clinical pathways and care guidelines and discharge plan.

Collaborates with the patient/ family, interdisciplinary care team, medical staff, payers, and post discharge providers to facilitate and coordinate patient specific evidenced-based care. Patient care is transitioned through communication and appropriate referral to the next provider of care.

**** 8am - 4pm
**** one weekend shift a quarter required
