Full time 35 hours per week

Must be able to travel to various locations throughout Brooklyn

Salary Range: $150,000 - $180,000

The Psychiatric Nurse Practitioner currently supports two Brooklyn Community Services Divisions – the Housing, Health and Homelessness Solutions Division which offers a variety of housing supports to people who are unhoused in Brooklyn including supportive housing programs, homeless shelters, and a street outreach program. Many of those clients experience co‑occurring mental health conditions and substance abuse disorders and Services for People Living with Disabilities which provides strengths‑oriented psychiatric rehabilitation programs devoted to helping adults increase independence and overall well‑being.

The PROS Counselor aids people living with persistent mental illnesses in developing life role goals, in managing symptoms, and in taking concrete steps toward a better life. The Psychiatric Nurse Practitioner will ensure the delivery of high quality mental health support in all aspects of service delivery to clients from vulnerable populations in accordance with funder and regulatory requirements; best practices;

BCS policies and Core Values; and the Code of Conduct for Custodians of People with Special Needs. This is a service delivery leadership position and the nurse serves as a role model to all stakeholders.

Responsibilities 1. Direct Clinical Service & Treatment

Clinical Assessment and Evaluation:
Conduct comprehensive psychiatric evaluations for program participants, including specialized evaluations for supportive housing applications.

Medication Management:
Provide routine monthly medication management appointments, including the authority to prescribe psychiatric medications and administer necessary psychiatric injections.

Specialized Assessments:
Complete psychosocial assessments for 2010E applications when case managers are unavailable.

Crisis and Support Care:
Deliver immediate crisis mental health support and provide necessary mental health support post‑discharge for members/residents.

Resource Navigation:
Provide necessary mental health community referrals as indicated by client need.

2. Documentation and Compliance

Clinical Documentation:
Ensure all psychiatric assessments, medication management notes, and missed appointment notes are accurately documented and submitted to the Program Director (or designee) by the end of each day.

Prescription Logistics:
Efficiently utilize the e‑prescribing system to submit and transmit prescriptions directly to the appropriate pharmacy.

Confidentiality:
Maintain the strict confidentiality of all program files and evaluation documents.

Program Oversight:
Oversee and ensure full compliance with all mandated protocols (e.g., HIV testing, harm reduction services) and funder requirements.

Licensure and Waivers:
Maintain all required regulatory documents, including essential waivers, licenses, and registrations (e.g., CLIA waiver, temporary authorizations).

Logistics and Inventory:
Oversee the distribution of syringes and Narcan, and maintain accurate, up‑to‑date inventory logs for all supplies.

Reporting:
Collect and submit all required monthly and quarterly reports/deliverables to the Program Director in the specified format and timeframe.

3. Team Collaboration and Professional Development

Team Meetings:
Attend required weekly team meetings (including specific 9:00 AM meetings) to review caseloads, bed rosters, and discuss program operations.

Case Coordination:
Participate in case conferences with the Case Manager and Program Director to ensure seamless, coordinated care.

Staff Training:
Ensure all program staff are properly trained on current program policies, procedures, and compliance standards.

Professional Development:
Participate in all mandatory training and meetings as directed by funding agencies and/or the Program Director.
