Role Focus

As a Full Time RN Nurse Assessor at Village Care, you will play a vital role in monitoring member/client progress towards their desired health outcomes through thorough assessment and evaluation. Utilizing your clinical expertise, you will assess and document the long-term community-based needs of potential members, ensuring compliance with the MLTC contract to determine eligibility. This position involves conducting in-person UAS-NY assessments and providing essential community-based training focused on self-care techniques and prevention strategies for frail populations.

You will perform comprehensive evaluations of clinical, social, and environmental factors, collaborating with consumers, families, and providers to develop tailored plans of care.

Your understanding of the complexities facing vulnerable populations will be crucial as you serve as an intermediary between members and healthcare providers, while effectively communicating Village Care Max 's features and benefits. You'll also engage with leadership to overcome barriers and ensure regulatory compliance, fostering a high level of care and support for each member's health journey.

Required Qualifications

To excel as a Full Time RN Nurse Assessor at Village Care, candidates must possess a robust set of skills and experiences:

  • A minimum of 2-3 years of clinical experience in home care or insurance plans is essential, with proficiency in UAS-NY required to effectively conduct comprehensive assessments.
  • At least 1 year of experience in home care, hospice, or field-based case management is preferred.
  • Ability to travel within a designated geographic area for in-person case management activities is required.
  • Excellent analytical and problem-solving abilities, with the capacity to navigate complex patient needs and develop tailored care plans.
  • Proficiency with standard corporate software applications (MS Word, Excel, Outlook, PowerPoint) and familiarity with proprietary applications.
  • Ability to work independently. Experience with MLTC, Managed Medicaid, MAP, or FIDA is a plus.
