Concord Nursing and Rehabilitation Center is actively seeking a Medical Records Clerk to join our Skilled Nursing Facility in Brooklyn, NY
.

Duties Include:

• Compile, process, and accurately account for all medical records for each resident
• Maintain confidentiality and security of all medical records
• Verify the accuracy and accessibility of files
• Maintain electronic health records databases
• Track and maintain data related to patient safety, disease patterns, treatments, and outcomes
• Release information to authorized individuals and agencies in compliance with regulations
• Audit and thin medical charts as required
• Schedule appointments as needed

Requirements:

• High School Diploma or equivalent required
• Basic computer skills
• Strong organizational, communication, and follow-up skills
• Experience in long-term care or healthcare settings preferred
• Friendly, dependable, and a team player
