Case Manager
Location: Brooklyn, NY
Position Type: Full-Time
Schedule: Monday–Friday, Standard Business Hours
Salary: DoE

Position Summary
We are seeking a compassionate and skilled Case Manager to support clients within the client services division through the development and implementation of holistic treatment plans. This role requires strong assessment skills, familiarity with medical situations, and the ability to coordinate services across multiple internal and external resources. The Case Manager will work closely with captains, clients, families, and community partners to ensure each client receives comprehensive, individualized care.

Key Responsibilities
  • Conduct thorough assessments of clients’ physical, emotional, mental, social, and financial needs
  • Develop individualized treatment plans in collaboration with captains and clients
  • Coordinate care with internal departments—pantry, clinic, entitlements, career services, and more
  • Collaborate with external providers and community resources
  • Advocate for client needs and ensure access to appropriate services
  • Provide supportive counseling, reassurance, and crisis guidance
  • Monitor client progress, adjust treatment plans as needed, and track all services provided
  • Maintain accurate documentation including assessments, progress notes, treatment plans, and case coordination
  • Attend allocation meetings for assigned clients
  • Participate in weekly individual supervision and staff meetings
Qualifications
  • Exceptional therapeutic skills for mental health assessment and cognitive reframing
  • MSW or Master’s degree in a mental health–related field (preferred)
  • Minimum 3 years of experience in case management and/or mental health services (preferred)
  • Strong organizational, writing, and communication skills
  • Proficient in Microsoft Office, Excel, and Google applications
  • Ability to learn and effectively use internal data tracking systems
Special Requirements
  • Familiarity with the Orthodox Jewish community
  • Ability to travel to clients’ homes as needed
