Hospice Care Coordinator
Role:
Hospice Care Coordinator at MJHS, New York, NY.
$46,640.00/yr - $58,300.00/yr
Responsibilities- Evaluates patients referred for Hospice services.
- Manages and facilitates the transition of patients onto the Hospice program in accordance with applicable law, regulations and agency policies and procedures under the supervision of the Director of Business Development.
- Bachelor’s Degree required; BS in health care related field highly preferred.
- Hospice or Health Care experience preferred.
- Microsoft Office (Word, Excel, Outlook, etc.) required.
