Duties & Responsibilities
Responsible for all aspects of assigned caseload, including meeting with clients living in Community Residence as needed but at least weekly. Make referrals to medical, mental health and/or substance abuse services as needed, and continue to involve providers in the clients care. Provide clients with supportive counseling. Make regular visits to client room, and monitor clients to ensure a safe and secure environment.
Teach and assist clients with activities of daily living. Complete client 2010 e housing applications, service plans, and psychosocial reports. Provide appropriate level of support including escorts if necessary for client. Ensure that all clinical files and reports are complete and up to date. Assist with intake and admission process. Participate in community meetings. Provide emergency first aid/CPR assistance when needed.
Related duties as assigned.
Hours
Full-time, 37.5 hours per week
- Sunday-Thursday 8am-4:30pm
BA preferred. HS diploma/ GED required and significant experience working with the substance abuse and/or mentally ill population required. Excellent writing skills. Basic computer literacy required. CPR training certification or willingness to take training class in CPR. Bilingual Spanish required.
- Vaccination preferred but not required

Program Description
Cecil Ivory House is a transitional housing program targeting formerly homeless clients who are either mentally ill or dually diagnosed as mentally ill and chemically addicted. Funded and licensed by the NYS Office of Mental Health with additional funding from the US Department of Housing and Urban Development, Cecil Ivory House is located in Harlem and has a capacity of 24 beds.
The mission of the program is to provide a safe and supportive environment where residents take part in rehabilitation interventions that will assist them in the attainment of their work, social and community living goals. A variety of services are available, including case management, substance abuse groups, medication monitoring, daily skills training and recreational activities.
