We are seeking a reliable and talented full-time Behavioral Health Care Manager, CCBHC to join our HUB and Inwood sites! Must be flexible to some work in the field. This is an onsite position.
Salary: $23-$25/hr

Position Summary: Provides care management for CCBHC patients, linking them to health and community services, documenting care, and coordinating with healthcare teams. Responsible for outreach, referrals, documentation, and client engagement activities.
Qualifications: Associate degree required;
Bachelor’s preferred in Social Work, Counseling, Psychology, or related field. 2+ years social services experience preferred. Bilingual in English/Spanish preferred.
