Role Description

This is a full-time role for a Care Coordinator at Tono Health based in Brooklyn, NY.

We’re seeking a Care Coordinator who’s excited to play a crucial role in ensuring an exceptional patient experience throughout a patient’s healthcare journey. In this role you will serve as a liaison between patients and other members of the patient’s care team as you coordinate appointments, manage referrals, and provide pre and post-appointment support. You’ll work directly with patients and their families, insurance representatives, pharmacies, laboratories, and physicians to ensure a seamless experience from start to finish.

Qualifications

• Care coordination and case management skills.
• Strong communication skills.
• Experience in care coordination and customer service.
• Proficient with Slack and G-suite, and comfortable learning an intuitive, easy-to-use EHR.
• You genuinely care about the well-being of patients and are passionate about being on the front lines of healthcare, changing patient lives.
• Excellent organizational and time management skills.
• Ability to work independently and collaboratively.
• Experience working in a healthcare startup environment is ideal.
