Social Work Director – Southampton, NY

Salary: $60,000.00–$65,000.00

• Assist new residents and families in the initial adjustment phase and ongoing transition to institutional living.
• Provide information to residents and families on resident rights and responsibilities.
• Interview residents/families to obtain social history and participate in discharge planning, development and implementation of social care plans and resident assessments. Ensure all charted progress notes accurately reflect services provided and resident response.
• Interpret social, psychological and emotional needs of the resident care team members and involve the resident/facility in planning objectives and goals for the resident.
• Refer resident/families to appropriate social services agencies as needed and arrange transportation to other facilities when necessary.

• Must have experience as a Social Worker in a Nursing Home.
• Strong written and verbal skills.
• Excellent communication skills.
