Position Summary: We have an exciting opportunity to join our team as a Clinical Pharmacotherapy Specialist – Adult Transplant. In this role, the successful candidate will develop and maintain a high-level practice in Adult Transplant Services, working under minimal supervision to troubleshoot challenges related to patient care, clinical program management, and staff supervision. The candidate will develop, implement and sustain clinical pharmacotherapy programs.

• Fulfill the core values and mission of NYU Langone Medical Center and incorporate them into the performance of duties.
• Maintain professional competency and certification.
• Identify and provide expertise for Medication Use Evaluations.
• Precept Pharmacy Residents; mentor Pharmacy students; support scholarly activity.
• Participate in hospital committees, as identified and assigned by the Director of Clinical Pharmacy Services.
• Document and report clinical interventions, adverse drug reactions, and medication errors.
• Develop and implement clinical practice guidelines that enhance pharmaceutical care and medication safety.
• Evaluate and report measures of patient encounter productivity and identify methods to enhance efficiency.
• Develop policies and procedures that optimize pharmacy operations while meeting accreditation standards and regulations.
• Contribute to the professional development of all pharmacy staff.
• Oversee quality assurance of clinical pharmacy programs to improve quality of care.
• Maintain a high-level clinical pharmacy practice. Participate in multidisciplinary bedside rounds, pharmacotherapeutic, pharmacokinetic and pharmaco‑economics consultation, medical response team, airway, stroke and acute coronary syndrome pager response, patient admission histories, transition of care involvement, medication counseling, and educational initiatives.

Doctor of Pharmacy degree from an ACPE accredited school of pharmacy. New York State Pharmacist Licensure. PGY‑I pharmacy residency training. PGY‑II pharmacy residency training or equivalent salaried professorship at a school of pharmacy. Board certification in Pharmacotherapy or another specialty required within 18 months of hire. Self‑motivation, strong communication, organizational, and interpersonal skills.

Required Licenses: Pharmacist License – NYS
