Care Coordinator Supervisor role at Children's Aid
Position SummaryThe Health Connections Care Management Program assists families with ensuring eligible children receive the medical and behavioral health care services they need to promote optimal health by linking, accessing and supporting families through service integration. The Care Coordinator Supervisor provides regular supervision to a team of up to 7 Care Coordinators (CC), individually and as a group. Oversees all Care Coordinators’ activities and ensures billable requirements are met and documented correctly so payment is received.
Provides coverage in Care Coordinator’s absence. Promotes Health Connections at community events and represents the program throughout Children’s Aid.
$64,350 - $70,000
Essential Duties- Understand and promote the organization’s Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
- Oversee Care Coordinators’ cases and care management activities.
- Provide regular supervision to up to seven (7) Care Coordinators; conduct regular and timely evaluation of CCs.
- Manage, review, and monitor maintenance of client records to ensure both internal and external quality assurance standards and timely completion in accordance with Children’s Aid and Health Home Policies.
- Ensure billing requirements are met and documented appropriately for reimbursement.
- Review and approve CANS‑NY for each of his/her supervises and complete CANS‑NY when needed.
- Compile reports as requested.
- Work closely with Quality Assurance Manager to ensure quality practice and service delivery, including preparation for audits.
- Participate in interdisciplinary team meetings for Care Plan development.
- Provide training to new staff and ongoing training to existing staff.
- Liaison between the array of Children’s Aid programs and Health Connections.
- Represent the Health Connections in the community and at other agencies.
- Help create linkages to community services for future resources for families as well as source of potential referrals.
- Field work as needed (possibility of evenings or weekends).
- Maintain confidentiality & HIPAA compliance.
- Work as part of the Health Connections team in all areas, focusing on meeting established targets.
- Participate in Program, Division and Agency wide activities (meetings, events, etc.).
- Attend trainings as required.
- Perform other duties as assigned.
- Master’s degree (LMSW/LCSW, LMHC, or related field) plus 2 years of supervisory experience required.
- Provide on call coverage as needed.
- Experience with Care Management a plus.
- Excellent organizational, time management skills, and attention to detail.
- Strong communication skills – verbal and written.
- Self‑motivated team player.
- Ability to multitask.
- Comfortable with field work and public transportation.
- Computer literate – Outlook, Word, Excel, GSI a plus.
- Bilingual Spanish a plus.
