Overview
Outpatient Psychiatrist role at Physician Affiliate Group of New York, P.C. (PAGNY) — two clinical sites in NYC Health + Hospitals Gotham Health:
Broadway and Cumberland, focusing on outpatient behavioral health and substance use disorder care. This is a part-time position engaging at two sites with direct patient care and collaboration with an interdisciplinary team.

- Direct Patient Care:
Provide direct care to patients referred or assigned to the behavioral health outpatient team, including medication management, appropriate and timely care, and comprehensive documentation. - Substance Use Disorders:
Provide direct care to patients referred or assigned to the SUD outpatient team, including medication management, appropriate and timely care, and comprehensive documentation; develop experience in MAT options including buprenorphine for opioid use disorders as applicable. - Complete psychiatric admission assessments with comprehensive risk assessments for new intakes and timely re-assessment according to policy.
- Outreach and documentation per policy; develop and review treatment plans with interdisciplinary team; ensure risk-appropriate care.
- Provide crisis intervention when needed; lead outpatient behavioral health crises and participate in medical codes until the Code Team arrives.
- Coordinate with teams to facilitate movement of patients to appropriate levels of care; conduct risk stratification and re-assessment; consider referrals to community resources (e.g., AOT, ACT).
- Collaborate with primary care and other medical specialists to provide clinical guidance and secure access to care; participate in discharge planning with referrals to alternative levels of care.
- Participate in and conduct interdisciplinary team meetings; participate in clinical tier reviews per policy.
- Provide supervisory oversight to clinical team members, including Psychiatric Nurse Practitioners and Psychologists.
- Participate in performance, quality assurance and risk management projects.
- May participate in collaborative case conferences, team meetings, and high-risk patient committees for review of quality of care and best practices.
- May participate in incident review committees and Root Cause Analysis; responsible for submitting, reporting, and reviewing incidents as indicated by policy.
- Participate in utilization review and management; maintain a focus on patient experience, access to care, increased market share, and financial stability.
- Assist in preparation for surveys; maintain awareness of regulatory standards for certification by various agencies.
