- Minimum 1 year of recent LPN experience in an acute care setting (home care experience preferred)
- Active NYS LPN license and current BLS certification
- Eligible to work in the U.S.
- Physical exam (within 1 year)
- PPD or Quanti
FERON (within 1 year) or chest X-ray (within 5 years) - MMR immunizations (within 10 years)
- Valid driver’s license (preferred)
