We are looking for an enthusiastic and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to become part of our clinical team. We are a trusted provider of comprehensive behavioral health services for residents in nursing homes throughout Plattsburgh, NY . This role is ideal for a practitioner who is deeply committed to geriatric mental health and motivated to enhance the emotional and psychological well‑being of older adults.
RequirementsPosition Responsibilities
- Perform detailed psychiatric assessments and evaluations for residents in long‑term care settings.
- Develop and implement individualized treatment plans that reflect each patient’s needs and diagnoses.
- Oversee medication management, including monitoring progress and adjusting treatments as clinically appropriate.
- Coordinate care with interdisciplinary teams such as physicians, psychologists, social workers, and nursing staff to ensure a fully integrated approach.
- Provide supportive counseling and psychotherapy to patients and, when appropriate, their families.
- Educate residents and caregivers on mental health diagnoses, medication options, and long‑term wellness strategies.
- Maintain thorough, timely, and compliant documentation within the EHR system.
- Participate in case reviews, team meetings, and initiatives focused on quality improvement and best practices.
- Active New York Nurse Practitioner license with certification as a PMHNP
. - Ability to independently manage a diverse patient caseload with strong clinical judgment.
- Proven competence in psychiatric diagnosis, treatment planning, and psychopharmacology.
- Strong interpersonal and communication abilities, with a focus on patient rapport and collaborative teamwork.
- Proficient in electronic health records and general computer usage.
- A compassionate, patient‑first philosophy of care.
