Counselor / Therapist

• New patient intake and orientation
• Conducts initial assessment of patient problems and needs and reports to clinical team to initiate treatment planning
• Conducts individual sessions
• Facilitates groups with patients
• Addresses patient's concerns and helps eliminate possibility of AMA
• Provides Case Management
• Provides tours for prospective patients and their families as requested.
• Documentation in patient medical record

• Must have one of the following: CASAC, CASAC-T, MH Permit, LMHC, LSW or LCSW
• License or certification, clean and active
• Minimum one-year experience in substance abuse
• Experience conducting group sessions
• Flexibility and willingness are great attributes

• $25.00 - $36.00 per hour

• Full Time
• Monday, Tuesday and Thursday 10:00AM - 6:30PM + Wednesday and Friday 12:30PM - 9:00PM

Hourly rate competitive with experience and location
