Location: Sunnyview Rehab Services, Vandenburgh Ave, Troy, NY

Job Overview: As a Physical Therapist in an outpatient setting, you will provide direct patient care, managing the full spectrum of patient assessment, diagnosis, treatment planning, and documentation. You will work with patients diagnosed with CVA, SCI, LE Amputation, Orthopedics, and Cardiopulmonary conditions as part of a multidisciplinary team.

• Formulate comprehensive treatment programs by evaluating patient’s past medical history, condition, impairments, disability, and functional status.
• Prioritize patient care needs.
• Assess rehabilitation needs and provide appropriate disposition, equipment, education, and treatment recommendations.
• Complete all documentation in accordance with professional, organizational, regulatory, and corporate compliance standards.
• Ensure all clinical and payer required documentation is concise, pertinent, legible, and compliant.

• Graduate of an approved Physical Therapy program with a Bachelor of Science, Master’s, or Clinical Doctorate.
• Current NYS Physical Therapy license and registration.
• Minimum one year of Physical Therapy experience (preferred).
• Excellent verbal and written communication skills.

$37.60 – $47.60 per hour, based on experience, skills, and education.
