The Phoenix Rehabilitation and Nursing Center is part of the Care Rite Centers Network and is looking for a talented and hard-working Licensed Physical Therapist (Full-Time) to join our ever-growing rehabilitation team.
The Phoenix is a facility with 400 beds and is proud of their state-of-the-art Rehabilitation Gym servicing 60+ short term beds. The Rehabilitation team consists of 30+ Therapists spanning 20 years of experience with backgrounds in acute, sub-acute, long term and outpatient settings, providing an excellent setting for learning and growth. We are a 7-day-a-week rehabilitation facility! In this role, you will have the opportunity to work with a diverse patient population on various sub-acute care services including post-surgical, orthopedic, neurology, post-stroke, cardiac, and much more.
Responsibilities Include for Physical Therapist:
The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services, which includes providing direct patient care to assess their medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation. The Physical Therapist will establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
Additionally, the Physical Therapist must set realistic and achievable goals for their patients, document and record the patient’s condition, and educate patients and families in appropriate physical therapy methods.
Qualifications/Requirements:
- Must have graduated from an accredited Physical Therapy college or university.
- Must have a valid professional license and registration for the state of NY.
- The Physical Therapist is responsible for overseeing and supervising Physical Therapy Assistants.
- Experience with long-term care and/or geriatric care is preferred.
- Well-versed in documentation writing, including evaluations, weekly progress notes, recertification, and discharge documentation.
- Collaborates with all disciplines to provide the best patient-centered treatment and care.
- Exceptional patient service skills and ability to demonstrate clinical excellence.
- Report to work on time, adhere to scheduled hours, and demonstrate professionalism at all times.
- Perform all other duties assigned by the Director of Rehabilitation.
- Compliance with confidentiality, HIPAA, and Federal Residents Rights.
Compensation: $41-$44/hr
