Position Summary: Education and Employment Specialists (EES) are members of FAP teams that include the Program Director, Supervisor, and Therapists/Case Planners. The EES provides comprehensive educational and vocational support, advocacy, and technical assistance to school‑age children and family members. They assist Therapists/Case Planners with case management tasks, conduct assessments, and connect families to services such as public assistance, housing, and community resources.
Each FAP Program assigns an EES to service all families in that program, including multi‑borough clients. The EES reports directly to the program Director.
- Promote the organization’s Mission, Vision, and Core Values in alignment with policies and procedures.
- Obtain and review copies of education and medical records for all school‑age youth in the home.
- Collaborate with Case Planners in educational planning meetings, including IEP/504, disciplinary hearings, and parent‑teacher conferences.
- Advocate for quality educational experiences through school visits, meetings with school personnel, student and parent dialogue, and monitoring school documentation.
- Serve as the YA WORC liaison; implement the YA WORC curriculum with eligible youth, including Career/Education Planning and co‑leading the Career Club.
- Enroll youth in SYEP and support successful job placements.
- Develop community resources for youth internships and connect eligible youth to these opportunities.
- Perform case‑management responsibilities, including:
- Assist families with concrete needs via the Office of Client Advocacy.
- Make referrals and schedule appointments to agencies and supporting services.
- Gather information on home and financial resources for new intakes.
- Provide emergency food, clothing, furniture, and school supplies as needed.
- Engage in outreach activities and cultivate partnerships with CBOs, schools, support agencies, and businesses to meet self‑referral utilization goals.
- Document all casework and collateral contacts in CONNX, PROMIS, and the shared drive.
- In rare instances, perform a case‑planner role for closing cases, completing FASPs and/or Plan Amendments.
- Participate in supervision, trainings, YA WORC training and technical assistance calls, and team meetings.
- Support families across multiple boroughs in the multi‑borough FAP Program, if applicable.
- Work flexible hours, including in the field, at CA office locations, and as needed to meet family needs.
- Perform other duties as assigned.
- Minimum bachelor’s degree in education, social work, or related field.
- Minimum one year of experience working with children and families.
- Ability to work effectively with children, youth, and families.
- Experience collaborating as part of a team.
- Knowledge of the D.O.E. and its Special Education programs preferred.
- Excellent oral, written, and organizational skills.
- Flexibility and ability to work under pressure and meet deadlines.
- Bilingual (Spanish/English) preferred.
