Role Overview :

As a key leader in our post-acute care services, you will:
Conduct clinical case reviews and provide quality care guidelines Develop and implement personalized patient care plans Collaborate with leadership to improve care coordination and intervention strategies Partner with pharmacists on effective medication management Engage with community stakeholders to strengthen partnerships Ensure timely completion of charts and pharmacy recommendations Mentor and oversee clinicians in the Albany region

Preferred Qualifications:

Experience as a Medical Director in a skilled nursing facility (preferred) Board certification in Family Medicine, Primary Care, Internal Medicine, Emergency Medicine, Adult Medicine, Post-Acute Care, Home Health, Hospice, Palliative Care, Urgent Care, or Hospital Medicine Position Highlights:
