What You???ll Do:
Conduct primary care rounds four days per week in the skilled nursing facility Diagnose illnesses, manage chronic diseases, and create comprehensive care plans Collaborate with clinicians, facility leaders, and staff to optimize patient outcomes Build strong relationships with patients, families, and the care team Mentor and guide nurse practitioners, fostering professional growth Lead clinical case reviews, sharing tools and strategies to improve care quality Represent the facility in community engagements to strengthen partnerships Develop care coordination strategies and targeted intervention plans with leadership

Qualifications:

Minimum 1 year of primary care experience (post-acute care experience a plus) Active New York medical license (or in process) Active DEA/prescriptive authority (or in process) Why Join Us:
