Job TitleMedical Specialist – Senior Practicing Specialist (Medical Specialist
2)
- Diagnose and treat patients within your specialized field.
- Advise and confer with members of other disciplines so patients receive comprehensive care.
- Serve as a senior practicing specialist within a recognized discipline of medicine.
- Assist in development or revision of medical policy and procedures to ensure quality care.
- Review lab work, specialty consults, hospital discharges, medical visit summaries, and diagnostic reports to assist the group home RN and treatment team with complex cases.
- Discuss care with hospital or rehabilitation facility providers to ensure continuity between the group home, hospital, rehabilitation setting, and community providers.
- Review and approve discharge summaries before accepting an individual back into the group home residence.
- Review medical information for new admissions to ensure appropriate care can be provided.
- Communicate with team members regarding medical issues and participate in team meetings as needed (in person or virtually).
- Communicate with family members and advocates to explain medical or treatment issues.
- Provide short‑term urgent prescription renewals when a community medical provider is unavailable.
- Serve on interdisciplinary treatment team meetings, committees, and work groups to improve individual services and outcomes.
- Provide after‑hours and weekend on‑call medical telephone consults on a rotating basis.
- Provide coverage of primary medical care for people living in a Comprehensive Adult Transitional Home if required.
- Provide medical expertise to Family Care Program and Community Mental Health Nurses as needed.
- Assist with physician coverage in sister regions when needed.
- License to practice medicine in New York State (or a qualifying limited license/permit).
- Certification by a Member Board of the American Board of Medical Specialties (ABMS) in your specialty.
- Not listed on any federal or state Medicaid/Medicare exclusion list.
- Ability to become an enrolled Medicare provider and obtain a National Provider Identifier (NPI) if required.
- Maintain medical license, limited permit, and board certification to remain in the position.
- For limited license holders: must obtain full New York State license within one year of receiving the limited permit.
- Position part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS) – non‑competitive appointment; no exam required to qualify.
- Permanent competitive status may be achieved within one year without an exam.
- Paid holidays: 13 per year
- Paid vacation: 13 days initially
- Paid personal leave: 5 days per year
- Paid sick leave: 13 days per year (may carry over)
