Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports they need to thrive. We offer more than 180 integrated programs in Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services.

We reach almost 80,000 individuals and families, including almost 13,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. Over 85% of our families live in poverty.

Beach 21st: A residential building developed and operated by Community Builders, located in Far Rockaway, Queens. The project has 23 residential units (5 singles and 18 families). CAMBA, Inc. provides on‑site services.

Position:
Case Manager

Reports To: Program Manager

Location: 1047 Beach 21st Street, Far Rockaway, NY 11691

What The Case Manager Does
  • Maintain professional relationships with clients and ensure client confidentiality.
  • Practice Universal Precautions/Standard Protocol & Procedures.
  • Comply with all federal, state, city, and CAMBA security and privacy policies to protect health information.
  • Review documentation confirming tenants’ eligibility for programs and make file copies.
  • Create and maintain tenant files.
  • Conduct initial intake and assessment of tenants and their families’ needs, and perform periodic re‑assessments.
  • Collaborate with tenants to prepare initial and periodic revisions of independent living plans, including short‑term and long‑term goals.
  • Assist tenants in attaining goals by locating community resources and making referrals to services within and outside CAMBA.
  • Work with tenants to overcome barriers to their goals and to advocate for themselves toward self‑sufficiency.
  • Recommend and implement strategies to encourage tenant participation.
  • Monitor tenants’ progress through scheduled telephone contact and/or face‑to‑face visits, documenting via progress notes.
  • Participate in case conferences.
  • Follow‑up with tenants and referral organizations about contact and progress.
  • Provide required data for weekly, monthly, quarterly, and annual reports.
  • May recommend closing of cases where tenants have achieved goals and maintained stability, are unwilling to participate, or become ineligible.
  • Act as tenant liaison/advocate with outside organizations on education, healthcare, housing, legal issues, etc.
  • Schedule appointments for tenants with referral organizations.
  • Escort tenants to appointments (educational, medical, social service, etc.).
  • Assist tenants in completing applications for benefits and entitlements, and may process applications on their behalf.
  • Follow‑up after goal completion to ensure stability.
  • Evaluate living conditions through home visits.
  • Prepare marketing materials for the program.
  • Prescreen tenants over the phone for eligibility and schedule intake appointments.
  • Input tenant data and progress information into the automated database.
Education, Experience, Licenses, and/or Certifications Required
  • Bachelor's degree (e.g., B.A., B.S.W.) or
  • Four years of relevant experience, or
  • Associate's degree and two years of equivalent experience.
Preferred Qualifications
  • Bi‑lingual in English & Spanish.
Other Requirements
  • Must be fingerprinted and obtain a Criminal History Check with the Justice Center for the Protection of Vulnerable People; maintain fingerprint clearance throughout employment.
  • As a mandated reporter, immediately report suspected abuse and neglect to the Vulnerable Persons Central Register Hotline at 1‑855‑373‑2122 and complete a CAMBA Incident Report.
  • Flexible hours may be needed occasionally for shift coverage until 8 pm.
Compensation

40,000 – 46,250 annually

Status

Full‑time (35 hours per week)
