Registered NURSE; Part-Time -Family Health Center of Harlem

Position: REGISTERED NURSE (PART-TIME) -Family Health Center of Harlem
Work with health care team providing care to LGBTQ unhoused youth age 16-24 receiving services from the Ali Forney Center (AFC) in Manhattan. Work in conjunction with Institute's primary care providers and the staff of Ali Forney. The nurse will be responsible for routine nursing duties including injections, outreach and care coordination with IFH and AFC staff; triage and assessment;

self injection teaching; promote and coordinate access to PreP and PEP; coordinate with mental health providers; coordinate planning for patients who are aging out of care; manage Rx prior authorizations and coordination with pharmacies; supply inventory and ordering; may facilitate patient health education groups.

* Demonstrates the ability to perform clinical/technical/service/administrative tasks.

Records, reports and interprets clients' responses and collaborate with appropriate members of the health team in the Family Practice.

* Identifies those components of the nursing regimen, which may be delegated to allied nursing personnel and provides the necessary teaching and guidance.

* Systematically and continuously assesses patient's physical, psychosocial and spiritual health status and knowledge base to determine patient's health care and educational needs.

* Triages all sick and walk-in patients.

* Establishes therapeutic and supportive relationships with patients and family.

* Assures proper cost-effective utilization of resources.

* Maintains an environment that is conducive to meeting patient care needs and that facilitates patient flow.

* Promotes and provides for patient safety in accordance with state and federal guidelines.

* Practices and monitors infection control.

* Monitors patient flow and reassigns staff as needed.

* Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area.

* Participates in the development of other staff members.

* Meets regulatory, licensure and annual health assessment requirements.

* Identifies learning strengths and needs.

* Utilizes learning resources.

* Attends in-service and continuing education programs.

* Obtains certification in area of practice.

* Successful demonstration of competence in area of practice.

Communicates pertinent information and changes in patient status to appropriate members of the care team and other health care providers.

* Effectiveness in supervising and mentoring LPNs, Medical Assistants, and Nursing students.

Qualifications

EDUCATION REQUIRED

AAS/BS in Nursing

EXPERIENCES AND/OR SKILLS REQUIRED

Experience working with teens/young adults

Experience with LGBTQ population

Knowledge gender affirming care preferred

Knowledge of HIV care and prevention preferred

Experience administering vaccines and phlebotomy

Minimum of one (1) year experience in an Ambulatory/Medical Surgery/ER setting preferred

Bi-lingual, Spanish preferred

Demonstrated ability to plan, make decisions, organize, and direct the work of others

Minimum of two (2) years nursing experience

Demonstrated ability to perform venipuncture and administer medications including immunizations

Demonstrated clinical, communication, interpersonal, and organizational skills

Demonstrated ability to problem solve using critical thinking

Demonstrated basic computer skills including typing

LICENSES/CERTIFICATIONS REQUIRED

Licensed Registered Nurse in New York State

Current RN Registration Certificate

Current BCLS

Current Infection Control Certificate

Current Child Abuse Certificate
