8:00 A.M - 4:00 P.M
This is a full-time position, working 40 hours per week.
Duties & Responsibilities
Purpose of Position:
Under supervision of licensed pharmacist, the Pharmacy Resident will be responsible for participating in a structured postgraduate training program designed to develop advanced clinical skills and enhance pharmaceutical knowledge. Hands-on experience in various aspects of pharmacy practice will be gained, including medication therapy management, clinical pharmacy services, drug distribution, and medication safety protocols. Ensures patient safety, optimal utilization of resources, service delivery and compliance with NYC Health + Hospitals, health care setting, and all other relevant regulatory policies and standards of care to improve outcomes and patient experience.
Essential Duties and Responsibilities:
1. Participates in clinical rounds and patient care activities. Engages in interprofessional rounds to improve patient care and establish an understanding of how a multidisciplinary team, including nurses, physicians, social services, and other healthcare workers, provide comprehensive care and support services.
2. Conducts patient interviews and counseling on self-management of patient health by advising changes in lifestyle choices and medication regimens.
3. Conducts medication reconciliation and reviews for assigned patients. Compares patient's medications orders to current medication regimen to avoid medication errors by utilizing medication reconciliation.
4. Contributes to medication safety initiatives, including error reporting and adverse drug event monitoring. Identifies and resolves medication-related issues, including medication safety, efficacy, administration, and drug interactions.
5. Collaborates with healthcare providers to optimize medication therapy plans. Conducts pharmacotherapeutic and pharmacokinetic consultation and use therapeutic drug monitoring to individualize medication therapy for patients taking drugs with a narrow therapeutic index.
6. Provides medication therapy management, including examining patient medical records, performing a comprehensive medication review and communicating with patients and providers, including outbound calls to patients to discuss their medications.
7. Assists in the development and implementation of pharmaceutical care protocols. Promotes clinically appropriate prescribing practices based on evidence-based medicine and national guidelines through various modalities.
8. Reviews drug monographs and literature to provide appropriate expert advice on drug selection, dosage, and administration.
9. Engages in scholarly activities, such as research projects and case presentations. Prepares education seminars and programs for healthcare professionals, tailored to the specific needs of the institution about current and relevant topics (i.e., fall prevention, pain management, and antibiotic stewardship).
10. Contributes to and participates in multidisciplinary committee meetings, pharmacy department meetings, quality improvement initiatives, and continuing education activities.
11. Completes assigned pharmacy practice rotations to gain exposure to various specialty areas, including but not limited to internal medicine, critical care, infectious disease, pharmacy administration and ambulatory care.
