Job Summary
We are looking for individuals to join our client's team as Case Worker. Under supervision, you will provide social services to clients/patients, assist with discharge planning and follow-up, coordinate aftercare appointments, perform data entry, accompany patients to screenings and appointments, and support resource linkage efforts.
Job Description
- Conduct interviews and make initial assessments and referrals.
- Manage cases of social service recipients.
- Make home visits to adults to assess needs and provide support.
- Investigate and verify information obtained from clients and other sources.
- Provide health-related and social services to clients.
- Assist social workers with discharge planning and follow-up care coordination.
- Minimum of 3 years' experience in social services, case management, or related field.
- Experience placing patients in nursing homes and sending out referrals.
- Knowledge of social service programs and community resources.
- Strong interpersonal and communication skills.
- Ability to work with individuals who have difficulty expressing their needs.
- Organized, practical, and able to work collaboratively.
- Ability to compromise, negotiate, and work effectively with others.
- Careport and EPIC experience preferred.
- Candidate will be fully trained within the department.
- Bachelor's Degree required from an accredited college or university
- Shift Time: 9:00 AM-5:00 PM
- Up to $43.00 per hour depending on experience
