Works with adults receiving or needing institutional care or protective services.
Identifies, develops, and implements social service plans for families and individuals.
Determines eligibility for these services.
May perform counseling and investigatory activities related to the work.
Experience with Careport and EPIC preferred.
Candidate will be fully trained with the department.
