About the Role

The Health Service Monitor (HSM) will function as the bridge between the Agency's Mental Health and Care Management Services and Enhanced Family Foster Care (EFFC) team. The HSM will work with the case planning team to identify children, youth and families who may benefit and be eligible for the services offered to assist with and expedite the referral process for children and their families.

The HSM will assist the EFFC Program by assisting with eligibility documents, enrollment forms and consents. The HSM will assist the EFFC program by ensuring children are scheduled for medical, dental, and mental health services. The HSM will facilitate appointments with the agency's Health Services department and track children's initial and annual, medical, dental, and mental health services as per ACS and OCFS requirements.

The HSM will perform clerical duties under the supervision of the Manhattan Regional Manager.

What You'll Do

Develop a process for screening and referring children and families to Mental

Health and Care Management Services.

Meet regularly with program leadership and case planning staff to discuss

identified youth and progress with referrals.

Attend case conference and case plannings where potential referrals are

discussed.

Work with the case planning team to secure eligibility and work on enrollment

for children with complex trauma needs and qualifying medical conditions to

Care Management services.

Explain the program to parents when needed.

Refer child for an eligibility assessment.

Assist with the completion of enrollment forms and consents.

Work with the care management team to ensure smooth enrollment into the

program. Work with the case planning team to determine children who are

eligible for CFTSS or Article 31 mental health services.

Refer child for an eligibility assessment.

Assist with the completion of enrollment forms and consents.

Create and manage a tickler that tracks all initial and annual child health

services. This includes medical, dental, mental health assessments, and care

management services. Ensure accuracy of tickler and update monthly.

Liaison with Health Services to schedule all children's medical, dental, and

mental health appointments.

Liaison with foster, pre-adoptive and child parent for appointment availability

and follow up with reminders to child's caregivers of child's scheduled

appointment date and time.

Ensure and track diligent efforts to schedule child appointments and share

feedback with case planning team on children who have missed

appointments.

Review child/ family medical consent forms for accurately prior to submission

to Health Services department.

Assist Health Service department with ACS medical audit preparation.

Conduct clerical duties, i.e. photocopy, emails, filing and process letters for

mailings.

Willing to travel within the five boroughs.

Maintain Adoption and Kin Gap case records

Performs related and similar duties as assigned by Manhattan Regional Manager

Your Qualifications

AA degree Preferred.

Strong engagement and collaboration skills

Experience in child welfare and/or mental health services to children and

families in understanding regulations.

Computer literate and strong knowledge of Excel

Initiative-taking and ability to work independently.

Excellent organizational skills, ability to multitask, meticulous.

Effective communication skills both written and verbal.

Ability to carry out assignments with attention to accurateness, thoroughness,

and timeliness and with minimal supervision.

Excellent interpersonal and customer services skills.

Additional Requirements:

* Authorized to work in the U.S.

* Ability to work in-person in New York City (NY) for at least 5 days or more per

week.

* Ability to travel to other Rising Ground sites (if required)
