MDS Coordinator - RN

Position: MDS Coordinator - RN full time
The Manhattan Division of The New Jewish Home is searching for a full time MDS Coordinator RN to join their team. This position sits in our 106th , NY nursing home location.

The RAUM Manage will:

* Assume MDS responsibility for a caseload to of post-acute patients and long term residents; scheduling; completing and coordinating completion of the MDS and RAI process

* Issue Notices of Medicare Non Coverage and Detailed Notices and SNF ABNs

* Determine if patients meet Medicare coverage eligibility on admission and monitor for ongoing coverage during patient/resident stays

* Work with IDT to insure best practice & compliance in RAI process

* RAC-CT AANAC accreditation preferred

* 3-5 years recent experience as an MDS Coordinator at a minimum

* Proficiency in current MDS coding rules and Medicare coverage regulations

* Valid License and currently registered as a professional RN in NYS
