Experience:

One to three years of endocrinology clinical experience in an outpatient setting. Diagnostic

Excellent communication skills. Interest in Population Health Management:
Display an interest in population health management. Responsibilities:
Comprehensive Patient Evaluation:
Review patient s history and perform a physical examination. Maintain complete and accurate medical records. Conduct physical evaluation and diagnostic tests to determine diagnosis and provide appropriate treatment and patient care plan. Endocrinology Treatment:
Diagnose and treat patients with a variety of hormonal diseases and conditions related to the endocrine systems. Provide comprehensive cost-effective diabetes care with available support of CDE/RD services. Recommend suitable management for the patient, including preventive health recommendations and habits. Prescribe medications and formulate ongoing treatment and disease management plans. Relationship-Centered Care:
Provide relationship-centered care, taking into account the patient s psychosocial and physical needs. Communicate effectively with the clinical team to ensure total delivery of quality care. Share on-call phone answering responsibilities with other Endocrinologists, presently on or about every six weeks rotation basis. Educational Participation:
Participate in medical staff educational programs and meetings.

Qualifications:

Degree:
Doctor of Medicine (MD), Doctor of Osteopathy (DO), or foreign equivalent. Fellowship:
Completion of two years of postgraduate medical training from an accredited fellowship program in the area of practice (e.g., Endocrine, Diabetes, and Metabolism Medicine).

Experience:

Open to both experienced and newly trained providers. Certification:
Board Eligible/Board Certified in both Internal Medicine and Endocrinology. License:
Must possess a current, unrestricted New York State license or be eligible for licensure. Registration:
Must possess Drug Enforcement Agency (DEA) registration or be eligible for registration. Enrollment:
Must possess active Medicare/Medicaid Enrollment or be eligible to enroll. Certification: BLS or ACLS certification required but both strongly preferred. Communication

Skills:

Must possess strong communication skills, both oral and written. Technology Comfort:
Comfortable with Electronic Medical Record (EMR) system and Microsoft Office. Medical Office Hours of Operation:
