Position Overview

We are seeking a compassionate and skilled Social Worker / Family Therapist (SW/FT) to join our Early Intervention (EI) program, supporting children and families in home and community-based settings. In this role, you will partner with families during challenging times by identifying needs, developing individualized support plans, and connecting families to community resources.

You will provide counseling, advocacy, and care coordination in alignment with Individualized Family Service Plans (IFSPs) and New Mexico FIT Program guidelines, helping families strengthen protective factors and improve outcomes for young children.

Key Responsibilities

  • Assess family strengths, needs, and priorities through interviews and ongoing engagement
  • Develop, implement, and modify individualized family support plans aligned with IFSP goals
  • Provide counseling, guidance, and education to families during EI visits
  • Coordinate and refer families to appropriate community resources and supports
  • Monitor family progress and adjust support strategies per NM FIT guidelines
  • Meet EI documentation, benchmark, and billing deadlines using strong time-management skills
  • Participate in Transdisciplinary Team Consultation Meetings (TTCM)
  • Collaborate with EI providers and multidisciplinary team members
  • Provide supervision and support to other therapists as needed
  • Maintain accurate, timely documentation of all services provided
  • Uphold ethical, culturally responsive, and family-centered practices

Qualifications

  • Master’s degree or higher in Social Work or Family Therapy from an accredited program
  • Experience working with children and families (birth to three preferred)
  • Strong advocacy, communication, and interpersonal skills
  • Proficiency with documentation software and computer systems
  • Ability to meet EI benchmarks and deadlines
  • Dependable, detail-oriented, and collaborative team member

This role is primarily home and community-based, requiring reliable transportation and the ability to travel locally. Compliance with Aprendamos health and safety requirements, including vaccination policies, is required.
