Responsibilities
  • Initiates, develops, implements, and makes necessary revisions to the plan of care in collaboration with the physician and other health care professionals involved in care.
  • Directly and/or indirectly supervises care provided by the home health aides and licensed practical/nursing staff, provides instruction as appropriate, and assigns tasks according to state and federal regulations. Also provides required supervisory visits.
  • Provides high quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care, and in coordination with other members of the health care team.
  • Accurately and timely completes comprehensive assessments (OASIS) including medication reconciliation.
  • Accurately and thoroughly documents patient visits per policy and payer requirements, and completes documentation in a timely manner per policy.
  • Make the initial and/or comprehensive nursing evaluation visit, ensure patients meet home health eligibility and medical necessity guidelines as defined by payer source, accurately determine primary focus of care, develop the plan of care within state-specific guidelines with the physician, and submit accurate documentation.
  • Communicates relevant information timely and effectively with appropriate agency staff (e.g., patient care issues, visit assignments, schedule changes, orders, OASIS data sets, coding requests) to coordinate care with other clinicians. Communicates with physicians, patients, and family members to ensure quality care and service excellence.
  • On time, completes training, compliant.
  • Maintains a positive attitude, is willing to help the patient and agency, and is open to learning new skills.
  • Is punctual for all meetings and actively participates in all meetings including case conferences; expects a camera to be on for video meetings.
  • Takes direction from the Clinical Director and Executive Director professionally and completes tasks assigned timely, including required learning.
  • Assists in the orientation of new agency personnel and serves as a preceptor to other staff and students.
  • Participates in survey readiness activities and performance improvement plans, including reducing unnecessary hospitalizations and improving patient safety to achieve positive patient outcomes.
  • Participates in on-call and weekend rotations as needed to meet patient needs.
  • Adheres to and participates in the agency's utilization management model.
Required Qualifications
  • Current RN licensure in state of practice.
  • Current CPR certification or ability to complete within 90 days of hire.
  • Proven ability to function in diverse home environments and with patients of varied backgrounds and needs.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
Preferred Qualifications
  • 1+ years of clinical experience.

The salary for this role will range from $60,200 to $107,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
