We have an for APRN or PA to work in Skilled Nursing Facilities in the Waterford/New London area. This position offers a flexible work schedule, autonomy, and a pay for performance environment.
The Nurse Practitioner or Physician Assistant will travel to Skilled Nursing Facilities contracted with PACT, LLC.
Responsibilities- Provide medical care to a complex patient population in short-term rehab and long-term care settings.
- Analyze patient’s medical history, medication allergies, physical condition, and examination to determine best care for acute and chronic medical conditions.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs.
- Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate for assessed medical conditions.
- Record in medical record the physical findings, formulated plans, and prognosis based on patient’s condition within appropriate time frame.
- Coordinate with the nursing facility care planning team in the development of comprehensive care plans.
- Communicate with patients and family members regarding admission, care plan, discharge needs, prognosis, and end-of-life care as needed.
- Follow established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
- Master’s or Doctorate prepared.
- Certified nationally as a Nurse Practitioner through AANP or ANCC.
- Valid Connecticut license as a Registered Nurse and Advanced Practice Registered Nurse, and certified as a Nurse Practitioner by an approved certifying body.
- DEA and Connecticut controlled substance license.
- Experience working with elderly patient population.
- Experience working with EHR.
- State license.
