As a Home Health Registered Nurse, you will lead comprehensive in‑home assessments, develop and adjust patient‑centered care plans, and coordinate with physicians, therapists, and social workers to ensure seamless, holistic care. Your responsibilities will include wound care, IV therapy, medication administration, monitoring of chronic disease management, hospice or palliative support as appropriate, and vigilant safety assessments to prevent falls and complications.

You’ll provide patient and caregiver education on disease management, treatment plans, medication safety, and home safety modifications, while documenting clinical progress in a timely, accurate manner. Telehealth check‑ins, symptom triage, and escalation protocols will be part of your routine, ensuring continuity of care even when you’re on the move. You’ll have opportunities to demonstrate clinical leadership, mentor new nurses, and participate in quality improvement initiatives that elevate patient outcomes.

This opportunity offers competitive compensation with a weekly pay range of $2,010–$2,116, based on assignment specifics, with guaranteed hours of 40 per week. The contract is structured for multi‑week engagement, with extension opportunities for high performers and teams that align with client needs.
