* Position Summary:

** Monitors, coordinates and evaluates the care and appropriateness of hospitalization; initiates referrals.  Participates in development, implementation and analysis of care delivery standards and outcome measures.

* Essential Duties and Responsibilities:

*** Completes Continuity of Care assessments and establishes appropriate plans by initiation of timely interventions through collaboration with patients, families and multidisciplinary care team respecting choice of patient
* Concurrently reviews clinical documentation in medical record to assure timely progression of care transitions
* Collaborates with multi-disciplinary team and informs them of variance from established care pathways to ensure timely and appropriate progression of care and eliminates barriers for next site of care transitions
* Assists in expediting care delivery to avoid delays in timely service provision
* Provides feedback to medical staff and conduct education as expert resource regarding care coordination, discharge planning needs and regulatory requirements
* Delivers CMS regulatory notices within CMS established time frames
* Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits
* Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct
** Required Qualifications**:
** Education and Training**:
* Currently licensed as a Registered Nurse in the state of Illinois.
* BSN preferred. 2-5 years previous Case Management experience preferred.
* Current CPR
* Relevant hospital nursing; hospital case management; insurance case management experience preferred
** Work Shift Details:
** Various, Registry Status, Hourly pay, Days, 2 weekend shifts per 6 week schedule, one holiday per year

The final pay rate offered may be more than the posted range based on several factors including but not limited to: licensure, certifications, work experience, education, knowledge, demonstrated abilities, internal equity, market data, and more.
