The A&R Intake Assessment Clinician is responsible for:

  • Assisting in the process of responding to referrals and other requests for services and information.
  • Providing technical assistance to the discharge planning process, under the direction of the Director of A&R.
  • Responsible for being the first contact with the community and therefore excels at providing resources, and support to callers and walk-ins.
  • Providing general support to the case management program.
  • Document and process inquiry calls according to Intake policies.
  • Assess and stabilize patients before discharge or arrange hospital admission.
  • Notify and assist the on-call doctor with patient interviews.
  • Obtain administrative approval for all admissions.
  • Complete admission paperwork following hospital policy.
  • Maintain an intake log with all necessary information documented.
  • Make follow-up calls to patients who fail to show up for appointments.
  • Demonstrate guest relations by example and provide counseling to employees in need of training.

Your Experience Matters What we’re looking for:

  • Education & Licensure: Master’s Degree (preferred) in a human relations field. Licensed to practice in the State of Illinois as a mental health professional.
  • Experience: Preferably one or more years’ experience in an intake/assessment setting within a behavioral health, human services department/clinic or hospital. A minimum of two (2) years’ experience with customer service.
  • Additional Requirements: A working knowledge of a Mental Status Exam. A working knowledge of the Addiction and Recovery field.

Compensation:
