Registered Nurse - Home Health Case Manager. Nurses with experience in any of the following areas are strongly encouraged to apply:
Catheter care and placement, Clinical pathway, Develop and implement personalized care plans, Educating patients and families, Navigator, Skin assessment and ulcer prevention, Utilization Review, or Wound and Stoma care.
- Shift(s) available: day shift and mid shift
- Job types available: full time, part time, and per diem
