Advanced Practice Nurse, Primary Care Center

PositionSummary
The Advanced Practice Nurse (APN) provides high-quality primary care to patients of all ages in our Keyport, NJ community. This full-time role focuses on evaluating and managing acute and chronic conditions, delivering preventive care and education, and supporting patients’ long-term wellness. The APN works closely with a dedicated clinical team to deliver compassionate, patient-centered care that strengthens the health of our community.
PrincipalResponsibilities
- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
- Provide palliative care or hospice face-to-face evaluation services within established productivity guidelines (if applicable to the role)
- Deliver patient and family education to support understanding of diagnoses, treatment plans, and preventive health strategies
- Monitor the quality, safety, and appropriateness of services provided
- Identify opportunities for new services or programs aligned with the organization’s mission and community needs
- Adhere to infection control, safety policies, and all regulatory guidelines governing primary care practice
- Provide supervision and support to staff in alignment with organizational leadership standards
Minimum Qualifications
- Active New Jersey Advanced Practice Nurse (APN) license
- Master’s degree in Nursing from an NLN-accredited program
- Certification as a Family Nurse Practitioner (FNP) with prescriptive authority; active CDS and DEA registrations
- Minimum of 2 years of primary care or ambulatory care experience
- Business or management experience preferred, with demonstrated leadership abilities
- Reliable transportation, including a valid driver’s license and personal vehicle
Working indoors. Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking. Client/customer contact

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $120,000 to USD $170,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
