Registered Nurse - Hospice

RN Hospice Nurse (Home Hospice)

Base Pay Range: $45.00/hr – $50.00/hr.

• Perform assessments to determine a patient’s appropriateness for hospice care, learning needs, and progress toward goals.
• Develop, implement, and evaluate patient and family plans of care based on evidence‑based knowledge.
• Maintain visit productivity standards and ensure appropriate utilization of services.
• Complete discharge, admission, and re‑visit assessments, transferring data promptly.
• Use age‑appropriate teaching methods for patients and families; evaluate and revise teaching plans.
• Ensure a safe environment for patients and staff; follow infection control measures.
• Demonstrate pharmacology knowledge and manage medication reconciliation.
• Attend monthly staff meetings, PI meetings, and interdisciplinary team (IDT) meetings bi‑weekly.
• Participate in Performance Improvement activities, review records, and gather data.
• Provide orientation/mentorship to new employees; cross‑train as needed.

• Current New Jersey RN license.
• 2–3 years experience as RN in home hospice environment.
• Valid New Jersey driver’s license.
• Basic Life Support (BLS) certification required.
• Experience using an EMR, excellent critical thinking and communication skills.

Mid‑Senior level.
