Key Responsibilities

Administer medications and treatments in accordance with physician orders and facility protocols.
Create and implement care plans that address residents’ individual needs.
Provide skilled nursing care including wound care, vital sign monitoring, and reporting changes in condition.
Maintain accurate and timely documentation.
Ensure residents' rights and privacy are respected in accordance with HIPAA and facility policies.

Qualifications

Current, valid LPN license in the State of New Jersey.
Skilled Nursing Facility experience preferred but not required.
