Overview
Interim Health Care is seeking critical care trained registered nurses for high-acuity pediatric and adult patients in central New Jersey for home health visits. These patients are recently discharged from the hospital and need visits a few times per week for teaching and assessment. Make your own schedule!
Types of patients seen include fresh LVADs, fresh colostomies, wound VACs, tube feedings, ventilators, PICC line dressing changes, routing lab draws, etc. Infusion in the future (chemo, milrinone, TPN, IVIG, etc) as we build our team.

- Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
- Work with a team of RNs, therapists and aides to deliver quality patient care
- Take patient vitals, perform assessments, document progress and report changes to the RN supervisor
- Assist the RN in carrying out the patient’s plan of care and teach self-care techniques
- Educate family on the patient’s plan of care, medications and home safety
- Collaborate with the interdisciplinary team to coordinate patient care and plan for discharge
- Graduate of an accredited practical nursing program and active RN license in New Jersey
- Minimum of 2 years of critical care experience
- CPR certification (must be achieved within 60 days)
- Able to pass background and drug screen
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing, strong communication skills and compassion
