Overview

Sub-Acute, Long-Term Care is a sector of healthcare exploding with potential and work-life balance flexibility (no on call or weekends). We are seeking a part-time (16-24 hours per week) or 1099 palliative care Nurse Practitioner for a float position between several centers located in eastern New Jersey (cities include Mendham, Parsippany, Cedar Grove, Ridgewood, and Fair Lawn). The hourly range for this position is $58 - $62 / hour.

Responsibilities

The Advanced Practice Provider (APP) provides primary and urgent care services to nursing center residents in collaboration with other medical providers. The APP works with Center Leadership to achieve the center’s mission of delivering high quality health care and everyday compassion. The Palliative Care APP specializes in palliative care for residents with serious illness, focusing on pain and symptom management, psychosocial support, and advance care planning, while aligning with patient goals and preferences.

The APP serves as a consultant within the interprofessional team, providing recommendations for the plan of care and guiding patient-family-centered decision making. The APP also educates patients, families, and the care team on risks and benefits of interventions.

  • Utilizes evidence-based practice and standards of care to manage patient care, including: assessment of health status, diagnosis, interpretation of labs and diagnostics, development and implementation of treatment plans in collaboration with the interdisciplinary care planning process, and active development of advanced care planning for all residents.
  • Follow-up and evaluation of patient status; completion of necessary documentation in accordance with policies, procedures and regulatory requirements; adherence to evidence-based practice guidelines.
  • Provide health education to patients and families; demonstrate continued professional competence through ongoing education, certification maintenance, and applying standardized care guidelines.
  • Support the development of nursing staff in collaboration with Center Nursing Executive programs; partner with Center Executive Director and Staff Development Coordinator/Nurse Practice Educator to identify and meet clinical education priorities; serve as a clinical expert for nursing staff and provide targeted clinical education as needed.
  • Participate in center-based quality initiatives and clinical initiatives (e.g., avoiding hospital transfers, ACO initiatives, and other quality improvement efforts).
  • Adhere to Coding and Billing Compliance Program and comply with Align Med Partners time-off and on-call policies; perform other duties as requested; support Align Med Partners clinical initiatives.
Qualifications

The Advanced Practice Provider must be a graduate from an accredited School of Nursing, licensed as a registered nurse, and certified as a Nurse Practitioner with a Master’s Degree in Nursing. A concentration in Older Adult and/or Family Nurse Practitioner (FNP) is preferred; other concentrations or specialty tracks will be considered based on experience and business need.

A minimum of two years of experience working with adult or geriatric populations is preferred. In states where practice allows, APRN with an unrestricted professional license and DEA registration for each state of practice.
