Role Overview

As a Registered Nurse (RN) at Grace Healthcare Services, you will be an integral member of our Interdisciplinary Team (IDT), responsible for assessing and addressing the comprehensive needs of patients and their families. Your role involves initiating appropriate interventions upon admission and ensuring a continuous, responsive plan of care throughout the patient's journey.

Key Responsibilities
  • Deliver skilled nursing care as outlined in individualized care plans, ensuring comfort and quality of life for terminally ill patients.
  • Collaborate with physicians, social workers, chaplains, nursing assistants, and other stakeholders to develop and adjust patient care plans as needed.
  • Educate and support patients' families and primary caregivers, providing guidance on care techniques and offering emotional support during this challenging time.
  • Serve as a liaison between patients, families, and other healthcare providers, ensuring clear communication and advocating for patients' wishes and needs.
  • Travel daily to patient locations and facilities as assigned.
Qualifications
  • Valid New Jersey or Multi-State Registered Nurse (RN) license.
  • Minimum of two (2) years of nursing experience in acute‑care settings such as medical, surgical, or oncology units.
  • Strong clinical practice with a focus on customer service, excellent communication abilities, and the capacity to work independently in a fast‑paced environment.
