We are in need of a full‑time evening and weekend Licensed Practical Nurse (LPN) for our trach and vent ALS client located in Tabernacle. Tracheostomy and ventilator experience is required.

• Provide one‑on‑one care for trach and ventilator clients.
• Administer medications and treatments as prescribed.
• Document patient care electronically.
• Collaborate with interdisciplinary teams to plan and deliver patient care.
• Support patient and family education and discharge planning.

• Valid nursing license in good standing.
• Current CPR certification.
• Tracheostomy and ventilator management experience.
• Commitment to delivering high‑quality home health care.

• 8, 10, or 12‑hour shifts.
• 1st, 2nd, or 3rd shift.
• PRN, Part‑Time, or Full‑Time.
• No weekend or minimum requirements.
