Visiting Nurse Association of Central Jersey Community Health Center (VNACJ CHC), a federally qualified health center in Monmouth County, provides accessible, high‑quality healthcare through preventive and primary care, wellness services, mental health counseling, prenatal care, and more across our four community‑focused locations. We are seeking a Behavioral Health Consultant / Licensed Clinical Social Worker (LCSW) to join our integrated behavioral health program at the Keyport Primary Care Center, where you’ll help expand whole‑person, coordinated care for the Bayshore community as part of a collaborative and innovative team, with occasional rotation to other CHC sites as needed.

About

the Role

You’ll provide behavioral health consultation and brief intervention services for children, adolescents, adults, and families within a fast‑paced primary care setting. Working alongside medical providers, peer support specialists, and care managers, you’ll help patients address mental health needs, substance use concerns, and psychosocial challenges. Our integrated model includes primary care, dental services, on‑site lab work, MAT, SUD treatment, and care coordination—all under one roof.

What

You’ll Do
  • Provide scheduled and same‑day behavioral health consults in collaboration with primary care
  • Conduct assessments, brief interventions, and crisis evaluations
  • Deliver services both in person and via telehealth
  • Offer psychoeducation in a patient‑centered, accessible way
  • Maintain accurate documentation in the Electronic Health Record and follow all billing/coding requirements
  • Identify at‑risk patients through screenings, warm handoffs, and referrals
  • Collaborate within a Patient‑Centered Medical Home model and work effectively with diverse care teams
  • Participate in meetings and activities supporting the integrated care model
  • Contribute to program evaluation and quality improvement efforts
  • Ensure compliance with organizational policies, state regulations, and payer requirements
What You Bring
  • Master’s Degree in Social Work and active New Jersey LCSW
  • At least one year of supervised clinical experience (community health preferred)
  • Strong verbal and written communication skills
  • Excellent interpersonal skills and comfort working with diverse populations
  • Ability to collaborate within multidisciplinary teams
  • Eligibility to participate with all public and private insurance payers
  • Bilingual Spanish‑speaking strongly preferred

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000 to USD $110,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add‑on incentives, special clinical compensation, retirement plans, and all other benefits.
